when should kidney biopsy be done in a patient with swelling of feet or body?

Swelling of whole body also known as anasarca, or swelling of feet can related to kidney disorder. Anyone having such symptoms should promptly seek the consultation of a nephrologist at the earliest. In my practice i see a lot of patients who actually present very late and in such cases the treatment is prolonged and responce to tretment is delayed. However in other cases of anasarca or pitting pedal odema, after ruling out other possible culprits, a 24 hour urine protein estimation is done, or a protein-craetinine estimation is done. If and only if a renal cause of presenting complaint is found, is a renal biopsy planned and done. however in certain cases like diabetic nephropathy, although kidney is the culprit a biopsy may not be mandatory.
