A 5 days old baby girl was brought by parents with an ultrasonogram done in 34th weeks of intra-uterine life revealing an intra-abdominal cystic mass in the fetus (origin uncertain, size 7.4x8.3 cm). The baby was born by caesarian section at term. An abdominal ultrasonogram revealed the persistence of the cyst and increase in size (8.6x 9.2 cm), still of uncertain origin. Laparoscopy was performed using three 5 mm ports, one supra-umbilical for telescope and two flank ports. The cyst was found to originate from right ovary. The inner lining of the cyst was removed after diathermy incision leaving the outer wall in situ. Opposite ovary was normal. A drain was kept for 24 hours. The baby was discharged on the 2nd post-operative day. The baby is doing well at two weeks after operation.
